zyrtec only addresses some histamines; and for me, since it was first a prescription, it stops being effective for almost anything after a while, and

I have to take breaks from it to get it to be effective again. (don't tell plp, but my doctor said that 20mg, 2 pills, doesn't make enough difference to bother and face possible risks - when it was a prescription, I went up to that and that's how it played out for me, for one anecdotal confirmation.)
you may need to try another kind of antihistamine in place of zyrtec.
the nasal spray may help with some congestion; but if it's really bad, then you may want to add a decongestant. since you're on other medications, though (iirc), I suggest you check with your doctor if you are ok taking something like Sudafed (behind the counter is the good stuff but it can be notsogood with certain medical conditions, like high blood pressure)
