The delay between 4 and 5 was him enjoying the success and taking some time off from the ASOIAF world.
It's less "oh god, I can't write" and more "how do I get all these pieces where I need them to be" and then hitting issues with timeline and character that need resolution to move forward.
He knows the big plot pieces and the end. The details of getting there in an enjoyable, logical fashion is where the long writing times come in.
