Maybe not applicable, but I tend to go out and document the crap out of things I'm working with when I run into that

It likely gives me a deeper understanding of the system in question but also lets me put together some documented processes which of course can then be 1) vetted by others 2) referenced when mistakes occur and then improved
ideally that would eliminate mistakes going forward as reality and the documentation converge
