I played with one guy who had a similar problem.

He might have had it happen 3 times in 4 games. It sounds like it had been happening quite a bit before I joined as well. Then played with someone else later who got to 8 or 9 at least once every game for 4 games. That includes getting to 8 and having his super once. Followed by 7 or 8 with a super and heavy ammo. As soon as he went to try and get the kills he was chased by a bladedancer into someone with Thorn. He was a bladedancer as well so he couldn't counter.
I've also failed to get it this time despite getting close a few times.
