it's 2013. Engineers are testing the final product and the emissions results look bad. They bring this up to their supervisor (or VP or whoever) who says "We still have to make the launch date. Just make it work." and the CEO is never informed. CEOs just don't have the time or interest in being involved in these decisions the way conspiracy theorists imagine. It's the mid-line employees who are doing this more often than not, I think.
