I think it explains the issue. I'm not confident Pagano would know that his gunner was injured or that the backup never practiced the play.

I'm much more troubled by the fact that they practiced it that much and yet the players STILL lined up wrong. A special teams coach should get fired for that kind of situation, and I'm not a FIRE THAT GUY kind of person. He's the one responsible for both issues.
Pagano's in charge of managing the live action. If he wasn't alerted to the fact that a backup who had never practiced was going in, he's absolved. If he knew it and still let that happen, yes, he looks much, much worse.
