There's probably been one since then but Kendall Langford got ejected for trampling a guy in 2013.

That game had a lot of fights, so he probably wouldn't have if not for the previous issues. That plays into your argument that it's super-rare as a one-off. If you look at the video above, it's definitely warranted there.
