Ooh, another good one: a Hydrogen bomb almost detonated in North Carolina only days after JFK's inauguration.

Three of four fail/safes failed. The fourth, which was later proven to be faulty due to manufacturing flaws, somehow saved everything.
A different h-bomb hit the ground at 700 miles per hour and basically buried itself forever.
