ST is smarter and more creative: How would you describe a situation where a person has mis-characterized something you have casually mentioned?

A colleague in another federal office has done this a couple times this week. I will say "I'll email you a summary of that conversation" and I'll get an email back from this person stating "You were going to send me a transcript of your conversation along with supporting documents."
Or on another occasion, I asked this colleague for a list of agency contacts that they'd already reached out to (without getting a response back). I said I'd reach out to the same folks and see if I could get a response.
This colleague sent me an email asking "Did your boss tell ___(some government official)___ do respond to our inquiry?"
I don't know if this colleague is doing to intentionally manipulate me or if they're just as stressed out, strung out, and mentally foggy as I am.
But I now have to send my direct supervisor a CYA email explaining what's going on (and warn some of my cow orkers to be especially careful in communications with this particular colleague).
I just can't think of a good, concise description for what the colleague is doing, other than ...
"I’ve noticed a trend with __(person)__ over the past week: ___(person)___ has a habit of later expanding and adding onto statements I’ve said."
(Then I provide a few examples).
Can you think of a better, more descriptive way of phrasing that?
