reviews, at the time anyway. ~$99 for 2 years (not good on unlocked phones, may be other restrictions) - actually less because of credit card cashback, $75 deductible. the deductible for having it under my home insurance was almost the cost of the phone, so that was no-go; and it didn't look like either of my credit cards would cover into the future.
my oldest sister's experience makes me think that it can depend on someone's history with phones - she pays at&t's silly highness; but she's needed two iphone replacements in less than a year, and there was no hassle, so it was worth it for her.
note about credit cards: if you pay your bill with one, it's worth it to check if they have something in place re: damaged phones. this article has a list, but it may not be comprehensive.
