It's still an expression of contempt, not of someone completely at ease with coexisting.

Press the people who use the word "lifestyle" and I think you'd find overwhelming criticism of the definition of marriage. Press them into a minority position and they'll flip from coexisting to claiming that that "lifestyle" is ruining society.
