that was awful, of course; but he also had physical pain issues - he reportedly needed double hip replacement for years,

exacerbated by his stage work while wearing heels and the stress that caused; but, because it couldn't be done without the chance of a blood transfusion being needed, he wouldn't do it. (as far as I can see in a quickety google, he never had it.)
