We know he didn't kill his attacker.
In the book and the show, Meryn Trant of the Kingsguard was sent to kill him. In the books, he's still alive, and in the show, he was until Arya ran into him in season 5.
So that leaves "he escaped to the black cells and took another identity, and Ser Meryn never told anyone"
