Totally depends on the plan. I just checked mine: Inside is a C&P of my basic work health coverage: (Dental is at bottom)

Vision Care
· Eye Exams - $75 during any 24 consecutive months. Coverage is available for
you (the employee) and your eligible dependents.
· Prescription Glasses - $200 during any 24 consecutive months. Coverage is
available for you (the employee) and your eligible dependents.
Pay Direct Drugs - Generic drugs which legally require a prescription unless such
generic does not exist
· Drug Dispensing Fee Maximum - $8.00 per prescription
· Drug Maximum - Unlimited
· Anti-smoking drugs - $300 per lifetime, subject to the Overall Drug Maximum
Professional Services -
· Chiropractor - $300 per calendar year, plus 1 x-ray per calendar year
· Podiatrist/Chiropodist - $300 per calendar year
· Osteopath - $300 per calendar year
· Massage Therapist - $300 per calendar year
· Naturopath - $300 per calendar year
· Speech Therapist - $300 per calendar year
· Physiotherapist - $300 per calendar year
· Psychologist - $300 per calendar year
Private Duty Nursing - Reasonable and Customary maximum
Licensed Ambulance Service - Included
Medical Equipment - Included
External Prostheses - Included - Breast Prostheses are subject to a maximum of
$200 every calendar year
Compression Type Stockings and Surgical Type Stockings - $25 per calendar
year
Braces (other than foot braces), Trusses, Collars, Leg Orthosis, Casts and
Splints - Included
Custom-Made Orthopaedic Shoes - 2 pair per calendar year for custom-made
orthopaedic shoes which are required because of a medical abnormality that, based
on medical evidence, cannot be accommodated in a stock-item orthopaedic shoe or a
modified stock-item orthopaedic shoe (must be constructed by a certified orthopaedic
footwear specialist and recommendation of either a physician, a podiatrist or a
chiropodist is required)
Custom-Made Orthotics - $250 per calendar year
