one of our testing environments, with a Test Patient with less than 3 PHI (Protected Health Information) data elements. I'm 5 9s (99.999%) sure it was a test patient but not 100% sure.

And by Shredder, I mean the sealed box which the contents of later allegedly goes to the shredder. I tore it up first before putting it in there.
