Went through all of that with both my kids. Transitioning relates to the hyper-focused aspects of ADD/ADHD as well as impulse control. The "fine at school" but melts down at home is also an effect of being "on" all the time at school to control/focus and then releasing in the safe confines of home.
We were lucky that my wife was diagnosed in her 40s with ADD which got us on the path to help the kids even though my son had sensory issues from a very young age that we knew about. There was more going on with him and with my daughter than just those but they were a huge contributor to the home environment. On the right meds, they're better versions of themselves, really the people they want to be externally but couldn't be because of what was going on internally...I wish I could understand or explain it better but it isn't the reality *I* live with except that it's the reality for everyone else in my house and I see the results of it daily...
