I only watched parts of it, but caught the reveal at the end (spoils) -- (edited)

The hospital scene takes place in the past, they lose one of the triplets during delivery, but the father happens to run into a fireman in the nursery who had just dropped off a baby that was left at his firehouse, so the dad adopts him in place of the one who didn't make it, and those are the three main characters in the present.
