the killing of these people, and the possibility of having to cut your kids arm off, was done slowly, and with a plan. He lured them into that position with planning, and did all this to get them to do his dirty for for them. It is obvious that it takes an extreme act to throttle Rick and make him comply. Just slicing an old man was not gonna do it.
