I would guess the answer is that even genes can "go bad" in identical twins assuming autism is a genetic disorder (I think it is)

if it's a % chance thing, then it is reasonable that one or both or neither would get it. The narcoleptic twins both happened to get it. The autistic twin did and the other didn't.
