She apparently told the producers she didn't want to come back before ABC chose not to renew. Since she's also a producer, she felt guilty about it when TNN picked up the show for a fifth season. So, she came back with the understanding that they'd eventually write her off. With a few episodes left in the season, she had (another) car crash. After saying nice things to a lot of other characters on the show, she died fairly suddenly. It was actually a swerve since she seemed to be doing that conventional "almost died but didn't" thing. Instead, it was "almost survived but didn't".
It's now her husband, the father of her children, and the daughters out dealing with the ramifications of her death.
