Government bureaucracy. And not the DC kind, although HQ is in DC.

People at my level know what's not working. Our managers know what's not working. Their managers know what's not working. We can't get the higher ups to listen. It can take MONTHS for a correction in our manual to be published, either because the author isn't sure it's necessary or the committee the author reports to can't decide, or they bounce it up to General Counsel who can take forever to respond. Some authors will make changes that are blatantly wrong and that wrongness becomes the standard to which we are reviewed.
Don't get started on the technology we use. Outdated, disjointed, incorrectly programmed...
