They stagger the start times. The people who are running solo always go first. Then when that wave is a ways down the road they let all the people with pets and strollers go. The start can be a little disconcerting if there are people/dogs who try to push past everyone at the start. Once everyone settles into whatever pace they are walking/running at it's pretty fun.
