First, he clips his leg on the rope doing the dive out of the ring. Then, he almost kills Omega by somehow missing the table on a pile driver.

It looks like the banner on the table slipped under his feet. Naito panicked and tried to grab the rail for support on the way down, which removed a lot of Omega's protection. It's good thing he cradled his neck the right way.

They're both super-lucky that wasn't much more serious. Naito looked *super* pissed when he first faced the camera afterward.
