Trump ran for POTUS most likely as a way to increase his brand; and most stories have him having zero expectation of winning as late as November 8th, 2016.
If she ever had an opportunity to say no, he assauged her with "I"m not winning, but our grandkids' grandkids will be richer than God with the publicity."
Then he won and he at least gets the opportunity to act on the job he had no expectation of having; while she's stuck with her do-nothing role with no practical ability to leave.
He's hated by many; if she walked, her son would be impacted and the fury of hate wouldn't be just those who hate her husband, but also those who blame her for abandoning her loved husband.

She chose a life with him, but in no way was this a realistic consideration.
