It depends a lot on the workplace culture as well - are people really encouraged to speak up?

In fashion where there tend to be a lot of creative geniuses with big egos who have control over people's jobs/influence over their futures, people might not speak up if they are going against the highest paid person in the room.
In engineering industries like nuclear and aerospace, companies often have a culture built around employees being empowered to speak up about mistakes, but that's because if something goes wrong with the product, people could die. But no-one's going to die from a fashion disaster.
