Steve broke Sam's hand to get his wings and be allowed back into heaven. I'm not sure but I think he was more in "guardian angel" form rather than walking around human.
I guess now they're both damned for eternity *together*, and that's how they should have been.
It just seems like there was another episode needed to wrap it up nice(r).
