It's not one way or the other. You start by fixing Obamacare and adding a public option with funding for people who can't afford it.

Introduce price controls on medication. Let that settle in and see what consequences that brings and how we can adapt for them. See if that's still leaving holes in the system and costs are out of control and then make more adjustments. Aside from mitigating the unintended consequences, these are actually things that might pass. M4A out of the gate will not.
Other consequences, by the way, would include older doctors who just decide to retire rather than work harder for a lot less money. A shortage of doctors as a result could also be a real issue. A step at a time is the only way you can do something this big. The first step was just getting Obamacare in the first place. Now we refine, improve and build on it.
