the death rate becomes higher if you don't have medical facilities to take care of the patients. you can say Italy had higher because of more elderly

or you could say the elderly were dying because the triage system selected a younger person, someone with family willing and able to provide care needed after discharge if they survive. (the two critical considerations after how sick the patient was)
