You most likely qualify. Oregon put together a video on how to answer the questions for the survey (my wife filed this week) instructing you to

basically lie on the survey. (due to covid-19 answer questions this way even if they aren't factual)
I hope the states help people fill these forms out, with better instructions for this pandemic.
