Doc probably watched Marty die dozens if not hundreds of times.
In the tunnel, when Marty is trying to get the almanac back from Biff, he ultimately ends up doubling back in the tunnel and Doc saves him from getting run down by lowering the flags.
No way that worked on the first try. Why would he even be at that end waiting? He wouldn’t.
Marty got killed and Doc had to keep going back until he saved him.
