Here's a template that I used for my 2 people moving to permanent WFH (they both moved to other cities in the midst of this).

• Work From Home should not interfere with daily availability during established work hours.
o Your established work hours are _______________________.
o If your established work hours need to be changed, must be communicated to supervisor 1 hour prior to the established work hour schedule.
o Must be available and working all day same as if working in an office.
o Your work calendar must be shared with manager for transparency. This is so the manager can see the subject line/purpose of the meeting.
o If taking longer than a 1 hour lunch, must notify manager and team leads + setup your OOO.
o No background noise – including children, pets, construction, etc.
• All meetings must be done via Teams and video enabled.
• All communication SLA must be met.
o All emails/chats need to be responded within 30 minutes.
o Internal emails/Teams chats from other teams with responses needed, 4 business hours.
o URGENT internal email/Teams chats with responses needed, 2 business hours.
o External emails with responses needed, 4 business hours.
o URGENT external email with responses needed, 1 business hour.
• You must have a private, secure workspace and will be responsible for maintaining security and confidentiality of company, employee and customer information. Should you leave the company, you will be responsible for the smooth transition of any work in progress, turning over company files and other XXXXX property assigned to you.
• If you are absent due to injury or illness, you are expected to properly notify your supervisor at least one hour in advance of your scheduled work time. Leaving a message does not qualify as notifying your supervisor. You must personally contact your supervisor. You must call every day that you are absent unless you are on an approved, continuous leave of absence.
