in this case it's more that carelessness on the part of the quarantine staff allowed transmission.

aside from the security guard who boned a quarantined person (and probably didn't spread the virus) there's not really much to point to that was particularly egregious. more your series of small mistakes that led to the current outcome.
there are several cases now of people who didn't have the virus facing a $50k fine/a year in prison for breaking or deliberately avoiding quarantine.
