1. It's a gimmicked deck. What you don't see are the instructions Blaine would give Ford to force the card Blaine wants.

2. The missing card then appears in a gimmicked orange.

Several pieces of fruit might be gimmicked to allow more choice, but the orange can be forced, too.
In this case it looks like the only fruit that can be "opened up" as Blaine said is the orange. The others aren't typically fruits you would peel to eat and the orange is right on top.
