On the other hand, if someone has been intubated for 3 weeks, I'm not exactly against trying ivermectin in an ICU setting.

I do think there's a difference between people taking it prophylactically and someone with severe disease wanting to try anything they've heard might work. If it was in a hospital setting where they could monitor and it was doing any harm, why not try? To me it's like a terminal cancer patient taking an unproven experimental treatment.
