My guess is the shot was one where the actor is looking directly into a camera and fires directly at the camera.

It was apparently during a rehearsal of the shot. So i would guess the cinematographer would be at the camera and the director was right behind her looking at the shot too for the rehearsal, which is how it hit them both.

i would say the wrong ammo was used for the shot (i actually thought blanks etc that could do physical damage had been made illegal, but i guess not.)
