It sounds like some of the confusion comes from the industry terms. My understanding of what happened -- (edited)

Is that they were using dummy rounds for close-ups (bullet, no powder in the casing) and blanks for fitting scenes ( powder, crimped casing and plugged with something to keep the powder in it). This is what they're calling live rounds for the exact reason that this happened - anything with powder in it is dangerous. They were testing for the lighting of the shot and Baldwin and the rest of the set were told the gun had dummy rounds, except it was the gun with blanks. While removing the gun from the holster to set up the shot, the gun misfired, sending some projectile from the blank out and killing the DP at close range.
This is only what I've pieced together from various articles and I could be wrong.
