I think surcharges or generally increased rates are the better option. First, while it may feel fair to allow anti-vaxxers to suffer for their

decision, it will ultimately wind up taxing the system with lots of people falling into bankruptcy and more hardship. Second, most of them don't think they'll get it, so they won't be incentivized to get the shot to avoid that outcome. Finally, if you raise their rates now, then they will feel it immediately and that may actually provide incentive to get the damn shot.
