I think it's a combination of FCC indifference and lack of public awareness.
It's an obscure, technological issue that is difficult to explain to the general public. Also, a lot of people have heard the "don't use your cell phone on a plane" mantra so much that it's basically a case of Chicken Little to them: "I've used my cell phone on a plane for years, even after the flight attendants have told me to turn it off. Nothing happened. I didn't make the plane crash. So what's the big deal?"
In terms of the government failures, it really is a classic case of "not my problem." The FCC sold off the spectrum in 2016 knowing full well what the potential problems would be. They essentially said: "Not our problem. It's up to the FAA and the aviation industry to fix any problems."
And then you had four years of the Trump Administration where the entire U.S. Government basically adopted a "fuck it" attitude toward regulation.
