It's a little odd that "third world" started out as "countries not involved in NATO or Warsaw Pact" but has come to mean

"cultural backwaters, cesspools, lawless places".
Though odd may be the wrong world since it isn't surprising that the NATA and Warsaw Pact groups chiefly saw/see unaligned countries as places to use as pawns.
