Yep. In Australia there's a Medicare system, but if you earn over a certain amount ($40,000 or so) you have to purchase private health insurance.

The private health insurance is tax subsidised, but you get to choose the provider. Employers don't provide any health benefits.
And you can also choose not to be insured, but that means you "choose" to pay a higher levy on Medicare
