this is the kind of investigative conclusion that always irks me with safety incidents.

like why were they allowed to alter an apparently incredibly crucial file, what were the procedures, why did they need to alter the file, etc. like if this one small thing can bring down the whole system, you're not doing a particularly good job of risk management.

I see it a lot in safety investigations where they conclude human error without looking at the systems and motivations that lead to the error.
