I don't think I've ever seen a place with middle and ju ior high.

But unless you count pre-school (pre-k now, I think) I don't remember any place with more than 3 levels. Well, there might have been a place or two where K was separate from the rest of elementary.
I mostly remember k-6 = elementary, 7-8 = junior high or middle school, 9-12 = high school. Some places middle school started at 6th grade, and I thought that was the difference between middle school and junior high.
