It does seem to be a recurring theme.

Lamb is always one step ahead, and River is always catching up. There's always one scene per season where Lamb tells this to River's face. Maybe River's just not that bright.

I do feel that Popov's scheme needed a lot to happen just right and it all basically did. When there's that many variables, it seems unlikely to happen exactly according to plan, and except for the shooting at Sr. Cartwright's house, it pretty much did.
