What is the point of filing an appeal with an insurance company? Are they *ever* going to accept the appeal/eat the cost due to their own mistake?

Very often, with more information provided, an insurance company will pay a previously denied claim.

I'm taking a guess that the first level people have strict guidelines to follow, while the appeals level has some leeway to judge a situation and work
