In college I was assigned to take photos at a basketball game. I saw a controversial thing happen right in front of me and didn't take the photo.

The coach was screaming at a ESPN cameraman and he had his hand over the lens. Apparently he didn't want them filming a conversation (can't remember what it was). I didn't know until afterwards that it was a major news story.
A month later I wasn't voted in as the next photo editor and I quit and changed to a different department. That photo would have probably tipped the vote to me. And then who knows?
