Well, what we found with our boys was that they'd stop a second and you could almost read their mind...

"If I do this I'll get a time-out. Is it worth it? Yep." and then they do it anyway.
And when they start doing stuff with the attitude that the punishment is worth it...that's when the punishment has to change.
