the biggest difference, i believe, is that in the US doctors are paid per service and (i think) in canada they usually are not.

So there is no financial incentive to see 100 patients a day instead of 20, or to order additional tests or more radical treatments if they aren't necessarily a better medical treatment option with better outcomes.
