This link says that as of 2003, only about half of Canadian doctors are paid primarily on a FFS basis. -- (link)

I suspect the number in the US is much higher. That said, I find it interesting that the article cites "patient flow" as a drawback of salaried doctors, whereas the big problem here tends to be 3 minute run-and-gun visits under the FFS model.
