It's not exactly on the fly. It just looks seamless.

They know when they're doing a reversal so the guy who is on the ground will drop his arms down to let the other guy's legs move freely around his body instead of catching them up high...
notice then he reaches up and grabs the dude's hips to stabilize him as the backboard for the forward flip. The guy on the ground is making sure that his head will be cradled inside the torso so when he initiates the flip, he's not actually spiking his head into the ground.
and then he does the forward jump/slam motion and plants himself.
It's beautiful execution by the guy in charge (the grounded dude).
