has he had work-ups since he was in the hospital about the mental/behavioral things? including someone checking medications and supplements and possible interactions and/or trying dose adjustments?
there are some things that just happen, obviously; but having those checks can sometimes help.
will never forget one of my brothers and I finding a drug interaction neither doctors nor pharmacists had caught when our mom started having behavioral issues, and the contraindication for two was in big letters on one of the drug sites. maddening and scary. I’ve since had experiences that shouldn’t have happened either. ugh.
